On January 1, 2027, Medicare's negotiated prices take effect for 15 more prescription drugs used by millions of seniors, including the semaglutide drugs Ozempic, Rybelsus and Wegovy. Here is every drug on the list, what it treats, and what the lower prices do and do not mean for your pharmacy bill.

How much Medicare and patients are expected to save
CMS estimates that if these prices had been in effect in 2024, Medicare would have spent about $12 billion less on these 15 drugs, a 44% net savings. About 5.3 million people with Medicare used at least one of them in 2024. CMS also estimates enrollees will save about $685 million in out-of-pocket costs in 2027.
What the lower prices mean at the pharmacy
The negotiated price is what Medicare pays the drugmaker. It is not your copay. How much you personally save depends on how your plan charges you:
- If you pay coinsurance (a percentage of the drug's price), a lower price usually means a lower bill right away.
- If you pay a flat copay, your cost may not change much, because the copay was already set.
- If you are in the deductible phase, you pay the plan's price until you meet your deductible, so a lower price helps from the first fill.
Every Part D and Medicare Advantage drug plan must include drugs with negotiated prices on its formulary, and manufacturers must make the negotiated price available to pharmacies. You will also be protected by the 2027 out-of-pocket cap: once your covered drug costs reach $2,400 for the year, you pay nothing more.
A note on Ozempic and Wegovy
Medicare Part D covers Ozempic and Rybelsus for type 2 diabetes. By law, Part D generally cannot cover drugs used solely for weight loss, but plans may cover Wegovy for an approved medical use, such as lowering the risk of heart attack and stroke in people with heart disease and obesity. Separately from negotiation, the federal government struck deals with GLP-1 manufacturers in late 2025, and Medicare is testing lower copays for some enrollees who meet clinical criteria for obesity treatment through 2027. Eligibility and details vary, so ask your plan and your doctor which coverage applies to you before you count on it.
The first 10 drugs are still discounted
Negotiated prices for the first 10 drugs began on January 1, 2026, and continue in 2027. That list includes Eliquis, Jardiance, Xarelto, Januvia, Farxiga, Entresto, Enbrel, Imbruvica, Stelara and NovoLog/Fiasp insulin. With the second round, 25 drugs will carry negotiated Medicare prices.
What to do during open enrollment
- List every prescription you take, with the dose.
- Check your plan's 2027 drug list to see what tier each drug is on and whether you pay a copay or coinsurance.
- Compare plans using your actual drugs. A plan that was cheapest in 2026 may not be cheapest in 2027.
- Switch, if needed, before December 7. Our guide to the seven Medicare changes to check before 2027 covers the rest of the list.
